A nursing care plan
commonly known as NCP outlines the nursing care to be provided to a patient in
the hospital, family or community. It summaries a set actions the nurse will
implement to resolve/support nursing diagnoses identified by nursing
assessment. The establishment of the plan is an intermediate stage of the
nursing process. It serves as a guideline of the nurse in the delivery and
evaluation of care.
